Please use this form to make your audition stipend request. Students are eligible for up to $300, and funds granted are to be used strictly for audition-related purposes (e.g. headshots, tap shoes, airfare, etc.)These requests should be made prior to purchase.

If this is your first time submitting for an Industry Award, you will need to submit a supplier form to the university. This form will be sent from the University's Shared Business Service Center (sbsc@ou.edu). If this is not submitted, you will not be reimbursed.

After approval, receipts need to be sent to mitzid@ou.edu within 14 days of purchase.
