Welcome to the OU-TU School of Community Medicine shadowing/observation application. Please read the details below carefully as you will not be able to submit this application without ALL required materials. You will be required to upload the following:

Immunization Form with proof attached (includes tetanus, diphtheria, varicella, rubeola, mumps, rubella, and hepatitis B)
Proof of negative TB skin test
Proof of Covid-19 vaccination
OU Physicians Confidentiality Agreement (completed and signed)
Volunteer Confidentiality Agreement (completed and signed)
Social Security card - signed (Undergrad summer interns are exempt from this requirement)

For proof of immunization attachments, if you have one document that lists several immunizations please upload it to all applicable fields as needed.

All of the uploaded materials are confidential and protected. Once the documents above are submitted, you will need to complete a criminal background check (at your cost) and HIPAA training through our HR department.
