2025 SGA Graduation Cord Request Form

Congratulations on your upcoming graduation! As a token of appreciation for your service in SGA, you are eligible to receive a graduation cord if you have served on the SGA Executive Board, Executive Council, or Senate during your time at OU Health Sciences.

Please fill out this form to request your graduation cord. Once your request is received, we’ll follow up with details on when and where to pick it up.
