Events Involving Youth

The University of Oklahoma has numerous campus locations and a variety of events (both in-person & virtual) that attract youth, whether sponsored by the University or by third parties. This form and accompanying Youth Protection Policy are meant to serve as guidance for these valuable experiences. Please note, this policy may not encompass each and every encounter with a youth and should be flexible depending on the circumstances and with the approval of either a supervisor in charge of the area or facility or by the Risk Management Department in coordination with the Office of Legal Counsel.

Any event that will involve youth should complete this form. This form may be saved and continued later.

*Completing this form does not act as a reservation for use of campus facilities. All reservations should be made through the respective space managers of Univ. of Oklahoma campus spaces.
