OTC Confidentiality or Non-Disclosure Agreement - ADDENDUM

A Confidentiality Agreement (CA) or Non-Disclosure Agreement (NDA) is established when two or more parties wish to disclose confidential information for a specified purpose. The CA or NDA outlines the terms and conditions of the disclosure of the confidential information. This form is required to be completed before a CA or NDA is drafted. All fields MUST be filled in before the form can be submitted.
