This form should be utilized to request OU-Tulsa Student Affairs support in proctoring exams for OU-Tulsa students with established accommodations between 8am - 5pm on weekdays. Please provide at least two business days notice for requests; if we cannot support the request, we will notify the faculty member via email, as soon as possible.
