All safety equipment that is required for project use at competition shall be used.
All participators have submitted waiver and release of liability forms to the REPF coordinator.
The REPF Coordinator, Faculty Advisor, and Team Officers have met and agreed that the vehicle/project is safe to operate.
That insurance coverage has been secured if deemed necessary for operating or using the vehicle/project.
That all University travel policies have been reviewed and will be followed while traveling to the destination
That all above information is accurate and correct. If anything changes, I will alert the REPF coordinator to this change.
