You are invited to participate in research designed to improve severe weather forecasts and warnings.

If you agree to participate, you will provide brief feedback on your use of Warn-on-Forecast.

There are no risks, benefits, or compensation.

Your participation is voluntary and your responses will be confidential.

Even if you choose to participate now, you may stop participating at any time and for any reason. Your data may be used in future research studies, unless you contact us to withdraw your data. After removing all identifiers, we might share your data with other researchers without obtaining additional consent from you.
